Heh, I guess the .htaccess lessons of the 1990s have been left by the wayside, shuffled into the dust bins of history. "Let's just use an md5 hash to name our file, that's secure enough, no need to try to secure permissions on it."(Also, wtf guys, you're already using a SPA router anyway, why are you not using that to handle access to the images, instead of just putting the images in a publicly accessible directory.)

There can be good reasons to offload serving of static content to a webserver directly. For example, if you want to keep your database relatively small (BLOBs in a database make it huge) or maybe you need a CDN.
That doesn't mean uploads have to be unprotected, though. Web servers like Nginx have modules on board that help secure static content:
