If there's a queue between the ordering intercom and payment window, and she's in the car immediately behind him, he could easily have got to the payment window after she ordered.

On a lot of McDonalds here, the order isn't a window but an intercom thing. The payment is the 1st window, the collection is the 2nd window.

People outside Deutschland tend to take a long time to order anything, not being able to make up their mind and changing the order several times. They also take a long time to find their wallet after they make an order and have to pay. By separating order from payment, you can parallelize making up mind with looking for wallet.

Hmm, I vaguely remember the drive in was like that in the distant past. But the second line was never open. Then they rebuilt it to use that for more parking space.

I'm sure you'd know more about that sort of thing than a giant corporation whose business relies on figuring out those sorts of efficiencies, however.
But since you're stumped: you can have different people taking orders, taking payments and handing out food at the same time. Which you obviously couldn't do with only one window.

The true efficiency only comes out at actual peak times, when they often have people standing outside taking orders instead of using the intercom and they're staffed up and making enough food to keep up with ridiculous demand.
The McDonald's near my old office was like this and it was impressive.

I've never experienced that, at McDonald's or anywhere. Peak time just means you're going to sit and wait in a long line.

Depends. I (used to) love flying out of Dulles at super busy times. Instead of making you take off your shoes and take out electronics, etc, going through security they'd have a dog out sniffing everyone as they walked by. The process of getting through the metal detectors / xray machines was super fast. Didn't have to remove shoes even. Of course, they'd have them all open.
But of course, only during known peak times. Off times you could get screwed because they weren't properly staffed for it.

The McDonald's near my old office was like this and it was impressive.The Chick-Fil-A near my office is that busy, it is quite the operation when they are in full swing.

The only thing I remember is that, unlike Houston, immigrations didn't have what felt like drill sergeants yelling at me to get in the cattle line.

It would help prevent injuries. I can only leave the shower so many times, while dripping wet, and walk to the kitchen to make myself another drink, before I seriously injure myself. The odds only go up the later my shower is in the evening.
