I learned to make a fist like that from my grandfather, who learned it in the Marine Corps. He said "whatever you do, don't tuck your thumb in and curl your fingers around it; it may feel natural but it's a good way to break your thumb if you hit hard enough."

That’s true however having your thumbs pointed out will get them broken and severe damage to the socket at the base.
This is a proper fist, thumb out of harms way as you should be making contact with your first two knuckles in line with the bones of your arm:

BTW: Trust me on this. I’ve been in more than my fair share of biker bar bust-ups and never damaged my thumbs. I’ve broken all the bones in my hand itself, by hitting a metal door when the fucker zigged instead of zagging, but the thumb was just fine.

Well, for that stance your body is way too frontal to the guy you're facing (should be more sideways), it's the left arm you should be raising as your left shoulder is the one forward and your right arm should be tucked way closer to the body with your right fist on a height with the solar plexus. The only time your arm should be facing down like your left is when you just swiped an attack to the side.
Also, your feet are way too close to each other, looks like you're putting the weight equally on both feet, that's just wrong. Either put the weight on the front foot (for increasing the range of your punch) or on the back foot so you can do a kick with your front foot. Also, your body should be way lower. I don't see much muscular tension either - this stance should evoke the feeling of a wound coil close to unsnapping.

Seriously, though, it was a photo op in a constrained space. It could have looked more realistic if the camera had afforded more room to space things out a bit. I actually had a stance closer to what you're describing, but he corrected it to look better for the camera.

The weight definitely looks like it's shifted to the back.

That's a good (and common) way to slow yourself down.
