You need to make that thread more generic, because I have to remind you that France required a written permission slip to get out of your house since the first lockdown one year ago. And also when the latest lockdown started a couple of weeks ago, all the talk was about how the permission slip now is two pages long...

@topspin since you fill it yourself, yes, that's pretty much it.
Though since it's an administrative form, it has to use proper administrative language.
So for example, you could to check the box that allowed you "to perform the acquisition of goods necessary to the professional activity and goods of first necessity (3) in establishments whose activities remain authorised (list on gouvernement.fr)", the footnote (3) detailing that this was "including acquisitions at no cost (distribution of alimentary goods...) and trips linked to the reception of social welfare and liquidities withdrawal".
That box meant you could go buy groceries.
There also was a box specifically for "summon by police or a court", that was added a few days after the start of the first lockdown. I guess there were policemen who summoned someone, and when they arrived to the police station fined them for not having a proper form .
