Status: dealing with consequences of a merger.
Specifically, my office is still treated as external traffic by the other offices, so the test versions of the web services I need are rejecting requests from me or anyone else here. The ticket to open the firewall ports has been open for weeks now. I was originally supposed to be taking this over at the start of June and haven't been able to run a single test case yet

We're smarter than this here. We just don't do merges at all. Every feature is implemented in a new copy of the code. And if someone happen to need more than one new feature on his versino, we just recode it or do some sort of partial merge, and ship it. After a succesful compile, of course, we're careful with stuff.

Nope, just woke up and I'm in rage because one of the f•°•§©¬s flew into my ear.
For those who shouldn't know, there are tactile sensors in my ear canal capable of overriding everything above layer 2 (upper-lower) functions, so I was rudely awoken when I found myself smashing my foot into the brick wall...

Company merger, not code merge. Between my interview and my job offer the company was bought by a multi national (they were aware it was coming when I was interviewed). I think I was the first hire after the change so I've been caught between the old and new processes for my entire time here.

Status: Trying to figure out how to phrase a "high level explanation of what happened and when we think the issue was introduced" when I honestly have no idea what was causing the issue.
We had an extraneous auth that was failing for this request and returning a 401, which caused the front end to instantly log-out that user. It turns out the auth was not necessary, so the "fix" was simply to remove it-- but I still have no clue why it was failing in the first place.

@blakeyrat Is "reproduced" supposed to be "introduced"? And I suppose this is for an incident root cause analysis?
If so, I would have it read "An extraneous authentication pass unnecessary to ensure application functionality and security but present nonetheless began (transiently?) failing at (date and time of approximate first report).
This extraneous authentication was introduced in (version or date of deployment to production where it first appeared or was made redundant).
Removal of the extraneous authentication resolved the issue. The root cause of the failure of that authentication is unknown, and it is (uneconomical or not possible) to investigate further.

I remember a similar bug:

For the first few iterations, it prints 4294967295 4294967295, and then it starts printing 4294967295 0.

@blakeyrat The totality of the document in our case is called a CAR. Corrective Action Report. The RCA is merely a section of it, and the rest focuses on identifying who is responsible and what action will be taken to ensure it never happens again. So it's very adversarial and you have to tread super lightly.
