I don't know whether I was ever asked about my sexual orientation in the US, but I definitely got some (optional) forms together with my contract which asked about various stuff to collect statistics on discrimination. I remember thinking "the best way to make sure you don't discriminate is not to ask me all that shit in the first place".

In theory, the interviewer and others who make hiring decisions will never have seen the equal opportunities part of the form. Of course, the race and gender ones are fairly easy to work out when you meet the person so it's slightly pointless, at least for some of the questions

Some of it is supposed to be for general monitoring. Asking questions like “are we advertising in the right places to get a mix of applicants that approximate the general figures for that field?” and “are our own hiring processes, interviewing, etc., putting off people, so that the mix of people we hire doesn't approximate the mix who apply?”. They're statistical questions used to check overall procedures, not evaluations of the individuals who are the data points.
You've got to ask the questions or you don't find out whether you've got problems as you have no data.

Most of that is for government contractors, so that one can say "we have x% Black people, y% veterans and so on." That gives them a leg up in competing for contracts (if the percentages are high enough, that is.) In theory it's a way to help minorities; in practice it's just box-checking.
