Sigh. I lost focus after this "wonderful" start where I got the rare quintuple-yellow followed by realizing the word I correctly thought was my most promising lead was actually the word I had used yesterday as my first guess; and I missed that fact for so long!

I fixated on trying to fit GHOUL after the first two guesses; but that obviously couldn't work without the L.

 

Having been put off balance, I made a couple of mistakes I possibly would have avoided if I was paying more attention

LUCID would have also been a possibility here. Meanwhile two columns to the left it was becoming pretty obvious it had to be CHAIR, which would have introduced the R.

 

The nail on the coffin was this obvious mistake

Completely ignored it had to have a D
