    It would be hard (and potentially risky) to change:

A hospital system can exchange information with hundreds of other systems at the hospital, plus exchanging medical records electronically with other facilities.  By far the majority of those systems you're sharing data with will have no equivalent concept; you need to figure the best way to send this out, and more importantly, accept it in correctly.

    2. That would easily confuse many users; having two fields which were almost always the same but occasionally different, and then having to remember which one gets the "M" and which the "F" would be prone to accidentally reversing the data entry.

    In the end, it's about erring on the side of caution.  If you ask someone about pregnancy unnecessarily, you might annoy them.  If you neglect to ask about pregnancy incorrectly, you might kill the fetus or cause lifelong harm, not to mention open the door for a massive lawsuit.   Hospitals are highly sensitive to that - it's not uncommon to be asked about pregnancy three or four different times over the course of having one procedure done.  It's not that they don't already know you answered the question ten minutes ago, it's that they're legally liable if they don't ask and something goes wrong.
