Fill-in prompts when opening Word .dot(m/x) document?

So, this is a question from my brother. He recently stumbled across a Word document (
.dot
- a template file, I think?) which yields a dialog with a form on opening the document. When you then fill out the form, the fields in the document are filled with the values from the form dialog.He'd like to recreate that experience. Basically looks like this:
Already searched for a bit but, as always, the proper keywords elude me (and thus the results are swamped with marginally related search results at best).
Anyone know how to achieve this? Or at least the proper term so I can look it up myself?

Might be ASK or FILLIN fields, which I think would automatically show the prompt if it's a .dotx file.

Huh. TIL. We've always generated a PDF (print-to-pdf) and then used Acrobat to create a fill in form on that. The problem with that is you need full Acrobat.

@loopback0 Yeah, but FILLIN only does a single field - the beginning dialog is where it's at. I think I'll have my brother send me the file so I can have a look under the hood.

@Rhywden then it's probably a UserForm with the associated VBA shenanigans.
edit: the screenshot wasn't clear at it's size and I'd not opened the bigger version to realise it was a completely custom prompt

I would think that it would have to be some custom VBA code under the hood doing that.

@TheCPUWizard probably beyond his brother's capabilities. Especially considering that he didn't know enough to see if it was VBA in the first place.

Nothing. But some other VBA can be quite worrisome.
