    You're a tiny bit right about the function cilia and nicotine's paralyzing effect, but that's all.  Tar will eventually be eliminated from the lungs, but tar really isn't your biggest concern.  "Tar" is really just a catch-all term for whatever makes it into the smoker's lungs.  Cancer is caused by damage to the DNA resulting from exposure to carcinogens in this "tar".  However, the biggest cause of lung cancer in smokers is almost certainly radioactive compounds such as polonium, which exist in minute quantities in "tar".  These radioactive compounds eventually exit the body (as does anything that enters the lungs), which is why smokers have a much higher rate of bladder cancer as well.  Anyway, most of the "tar" tends to build up in bronchial passages and never even gets to the alveoli.  This is why most lung cancer in smokers is of a particular variety (non-small cell squamous carcinomas) because it develops further up in the respiratory tract.

     

    Ten percent of people who are diagnosed with lung cancer never smoked in their lives.  Most develop lung cancer resulting from exposure to radon or asbestos.

    Not really sensible. Probably more of a PR thing. Yes, there's a tiny percentage of (1/6000) who get lung cancer from second smoke, but that's only trapped in a room with people smoking many hours every day for many years. It's absurd to think that someone catching a whiff of smoke smell as they walk up to the building is going to be harmed by it.
