Flagging a topic multiple times fails

Flag a thread (for example) as off topic. Then try flagging it as spam.

I didn't get an error, but once I flag the post (as Inappropriate or Off Topic) I can only reflag it as "notify tufty".

Undo flag, flag the other, undo, reflag both.

Ah. I never got the row of notifications like your screenshot, but I did get an error.
"Sorry, an error occurred" or something equally helpful. Probably FLAG_NOT_FOUND.

I generally update around 0800UTC on weekdays with occasional other updates at other times/other days.

Alternatively, course, it appears other non-mod admins on here occasionally decide to secretly update the site software without notice/logging it.
These I have no control over and the only hint I get is if the version number changes in docker from the last time I updated it (Staff board - most of you won't be able to get there.)
Or I get a totally unexpected "This site has been upgraded" modal when I get to work in the morning, as happened today.
