    It is not just that. At the companies I have worked for, the insurance company would send a bill to the employer every month to reimburse the insurance company for the payments made to providers. The insurance company does not itemize the payments, and does not say which employee they were for. But the employer pays less to the insurance company the less the employees (and their dependents) incur medical expenses.

    My employer genuinely cares about our health (small company). They just announced a "Biggest Loser" competition and the company is putting in about $1000 for the top three. Unlucky for me, they didn't do it last year and I just went from 40% body fat down to 8%.

    Previous job kicked in an extra $500 per year to your health insurance premiums if you signed up for a service that periodically sends out health surveys and basically pesters you about your health.
