They won't care if I went behind the system's back to fix a software fault.
I find it difficult to believe auditors would not care about that.

The notes field is not the work the clinicians did. It's a view of the work. With a big. Which I fixed.
The auditors would very much care that what clinicians see on their screen (i.e., what's in the notes field) does not match what is getting printed out, though. That what is on their screen is incomplete. Etc.

Is there even a point in persisting the field? Why not just have a view which includes it, or concatenate the relevant fields in the application?

Because that would be doing it right. Someone sometime didn't do that. @Captain is left dealing with the consequences, and can't stop the world to put things right.
