It started as a throw-away joke, and was effectively treated as such by characters within context, but expanded into something more like reality in one of the DLCs (reality being rather flexible in that DLC, as it is a game within a game where the inner game is driven by someone's overactive imagination, which is one of the truly great things about it). The name was picked by one of the final bosses of the main campaign to try to insult the players, but in a way that makes you think he is a goofball as well as a jerk.
It all sounds convoluted. It is convoluted, but in a good way.
