Formatting, can you do it?

Those fields aren't aligned horizontally OR vertically.
Worse, this is a recent change--it used to have proper formatting, and not too long ago.
It's busted in both Chrome and IE, I haven't checked other browsers.

They probably used to be in table cells, then someone probably removed the cells because they heard they should use divs instead.

The hilarious thing is there's another way at the form. That's a timekeeping app, and you can get to it via a URL that ends in &company=yourcompanyid. When you do that you get a customized version of the form that obviously doesn't ask for your company, but is formatted correctly.
And the timesheet page is formatted more or less sanely, so it's not as if they don't know what they're doing.

You can technically arrange
<div>
s in a table layout withdisplay : table-cell
, etc. Of course that would still get you some dirty looks from those hipster designers that didn't spend 10 years learning how tofloat : left
their shit for nothing, but meh, whatever.
