That happens when the kernel rejects the USB charger as High-Power, but doesn't notify the OS that it's accepting current at 200 mA. Since Android knows it's plugged in but not receiving data, it assumes it's an AC adapter, and lo, we get things like this!

Luckily a few more reboots usually helps, so long as you reboot without the charger plugged in.
