As far as I can tell, this happens if a topic is new and then you navigate around in Discourse.
I think entering a topic and then going back either via the back button or clicking the Header makes Discourse somewhat confused. Now the topic is new, because you haven't read it but it also has new posts (that weren't there the last time the topic was new), so it displays that number as well.

According to His Unholy Jeffness, using the back button is Doing It Wrong™, unsupported, should not be expected to work, and may result in nasal demons.

This is Doing It Right™, supported, and should be expected toworkdiscowork (but may also result in nasal demons, because Discourse).

Edit: just seems to be font sizes, and that's just different...
