"Periodically, we send out reminders to the users to check their personal information and update it if necessary to make sure it remains current," says a pilot fish on the scene. "I sent one such e-mail with the standard closing telling users to contact me if they have any difficulties or questions. I received a reply from one of my users who simply asked, 'Who's my manager?'"

For example, people can have an informal leader (i.e. the most experienced coworker), an official team leader, a project leader and a head of the department; all of them could be described as the "manager" but the employee might not know which one to put into the AD.

Yup. My company directory entry lists a guy I've never even met before as my "direct manager". Never mind the other 7 bosses I have to report to.
