Not discoverability, consistency. MS has always been awful at UI, but at least the feature is there, and visible.
Now, if it was only made apparent by pressing a key (or better, an obscure sequence of keys), then you could start comparing it to discourse.

Yes, I think that's right. The positioning at least maybe makes some sense. You only go forward after you've dealt with the content of the dialog, but you can go back without it. Or something...combining up/down with back/next concepts it makes intuitive sense to me, even if it's difficult to verbalize.

When speaking of legacy dialogs: hdwwiz.exe is still included in Windows 8 (although no longer linked from anywhere) and it allows you to add all kind of non-PNP hardware (like for example additional loopback adapters) without detecting anything before.
